It is mandatory to fill out this form if you wish to receive a stipend for the event/activity you are signed up for.  Arts Ottawa must first confirm that you are eligible to receive a stipend and provide you with the amount.  Once approved, the stipend will be sent within 30 days of completion of this form or your confirmed participation in the event/activity (whichever is longest).
